This Privacy Policy describes how Hustedt Health PLLC, doing business as Andronyx Precision Men's Health ("Andronyx," "we," "us," or "our") collects, uses, shares, and protects information when you visit andronyx.care, submit a form, or interact with our services. This policy covers website-collected information generally; for information protected under the Health Insurance Portability and Accountability Act (HIPAA), see also our Notice of Privacy Practices.
Information we collect
Information you provide to us
When you complete our New Patient Intake, service-specific screening forms, or otherwise communicate with us, you may provide:
- Identifying information (name, date of birth, email, phone number, mailing address)
- Health information (medical history, current medications, lab results, symptoms, treatment goals)
- Payment information (card on file collected via Stripe; we do not store full card numbers on our systems)
- Identity verification documents where required (e.g., government ID for telehealth verification)
- Communications with our team (email, secure messaging, voicemail)
Information collected automatically
When you visit our website, our hosting provider automatically logs basic technical information including IP address, browser type, pages viewed, and timestamps. We may also use privacy-preserving analytics to understand which pages are useful. We do not use third-party advertising trackers.
How we use information
We use the information you provide to:
- Provide medical care, including review of intake forms, screening, lab interpretation, and treatment decisions
- Communicate with you about your care, appointments, and prescriptions
- Operate, maintain, and improve our website and services
- Process payments and manage memberships

Third-party services we use
Andronyx uses a small set of vetted vendors to operate our practice. Each is a HIPAA Business Associate where applicable, with a signed Business Associate Agreement (BAA) in place:
- JotForm (HIPAA tier) — collects intake and screening form responses securely
- Stripe — processes payments and stores tokenized payment methods; we do not see or store full card numbers
- Charm Health (Charm EHR) — our electronic health record system, used to store and review your protected health information
- Compounding pharmacies — pharmacy partners we coordinate with to fill your prescriptions; they receive only the information needed to dispense your medication
- Netlify — hosts the website you are reading. Netlify does not receive PHI submitted through our intake forms; those are submitted directly to JotForm.
How we share information
We share information only:
- With your treating providers and their authorized clinical or administrative staff
- With the vendors listed above, only to the extent necessary to deliver the relevant service
- With other healthcare providers when you request a release or when continuity of care requires it
- When required by law (court order, subpoena, mandatory reporting, public health authorities, or law enforcement under valid legal process)
- To prevent serious imminent harm to you or others, where permitted
We do not sell your information. We do not share your health information for advertising or marketing purposes without your specific written authorization.
Security
We use administrative, physical, and technical safeguards designed to protect your information, including encryption in transit (TLS/HTTPS), access controls limiting who on our team can view records, audit logs where applicable, and signed Business Associate Agreements with vendors that handle protected health information. No security system is impenetrable; if we ever experience a breach affecting your information, we will notify you in accordance with applicable law.
Your rights
Depending on your state of residence, you may have rights to:
- Access information we hold about you
- Request correction of inaccurate information
- Request a copy of your medical records
- Request restrictions on certain uses or disclosures
- Request communications by alternative means or at alternative locations
- Receive an accounting of certain disclosures we have made

Cookies
Our website uses minimal cookies, primarily for basic functionality. We do not use cross-site advertising cookies. Most browsers allow you to refuse or delete cookies; doing so will not prevent you from using our website.

Our services are not directed to individuals under the age of 18, and we do not knowingly collect information from children. If you believe a child has provided us information, please contact us so we can delete it.
